Correcting a birth certificate

To correct information on a birth certificate, you will need to return all Registry-issued birth certificates in your possession, with a signed cover letter stating what information needs to be corrected and what the correct information should be.

You will also need to provide evidence of the correct details. The Registry will not correct a birth certificate without this evidence.

The Registry generally does not charge a fee to correct existing details on a birth certificate. However, you may be charged a fee to add certain details.

Please contact the Registry for advice and assistance in providing evidence for a correction.

Adding details to a birth certificate

You can apply to have the details of the biological father or the parents' marriage added to a child's Victorian birth certificate after the birth is registered.

To add these details, please contact the Registry to arrange for a form to be sent to you by mail.

You will need to complete the application form and return it to the Registry with any previously issued birth certificates. You will also need to provide proof of identity documents and the required fee as listed on the application form.

If you want to change your child's name you will need to make a separate application to register a change of name for your child, which will incur an additional fee.
